So was Cobb...spoilers

It seemed to me that Bill's twisted mind made him up, that this was what the film's ending was trying to say. Or was it all real, and Cobb & the Blonde really set him up completely??!

Can anyone help me out? This film confused me.

The blonde was setup by the Bald guy who used Cobb to kill her. The bald guy hired Cobb to kill the blonde and setup someone to take the fall. That way if the evidence of the bloody rug is found, the young man will be implicated in that murder as well.

The blonde setup the young man, only to add doubt to what she thought was the robbery and murder of an old woman that may be pinned on Cobb.
