Was the Dr. the girl in the beginning?

Someone told me that Dr. Russell was the girl from the beginning and that the reason she had a heart problem was because of the accident. I dont think thats true but that would be an incredible and awesome twist. That would make me love this movie even more!

No, Agnes was a girl who was in the same orphanage as Simon and died when she fell off the ledge. I think he flashed back to Agnes because he loved both Agnes and Emma and he didn't want to lose another woman he loved.
