The light was definitely added afterwards and only that specific light turns on exactly as he turns around.
It may have been a reference to the two earlier conversations where Gerald confesses that there have been times when he has trouble controlling his erection, such as when he was in school and was at the pool with lots of girls around. He was afraid that when they stripped that he would get excited, and maybe the light is a suggestion that he did. I just watched this again last night, and had never paid much attention to that plot point before, but it makes sense that the light could represent that.
