shareThe doctor's probably never been threatened like this before, so, on the spot and no doubt panicking, he chose the strong possibility of dying after the procedure over the certainty of dying before it. And the procedure probably takes a while, so perhaps he hoped that somebody would discover what was going on before Troy and his thugs got around to killing him.
Anyway, point is: hostages in real life are more than capable of convincing themselves that there's a chance they'll live even when there's none.

Because he wouldn't have just killed him if he didn't, he would have killed him slowly and painfully. When Archer is going through Troy's file on computer after he's captured, pause the movie and read them. He did some horrible things.

@farmerphil77 Dr Walsh knew that there would it be 100% if he would have gave to Castor Archer's face, that Castor wouldn't let him alive along with the other people who knew that Troy is disguise as Archer, cause somehow or soon or later they could have told the truth to the Authoritys specialy knowing that Archer is in prison disguise as Troy for no reason that's why Castor kill him along with the rest who knew the truth i think you know exactly what i'm talking about.

shareTroy either threatened to torture the doctor or go after his loved ones. So the doc got to choose between dying a relatively painless death or painful one including his family.
shareIt's very possible that Castor completely lied and said that the doctor's life would be spared only if he gave him Archer's face, and that the doctor was naive enough to believe that.
