Is the Mrs. Elton's very odd accent a regional one, or supposed to be an affectation of sorts?

It's supposed to be a Bristol accent.

Yes, Mrs. Elton is from Bristol, and her regional accent is a mark of her less than elegant breeding.

Interestingly, some people think she sounds American, but I don't think that was intended. The hard "r" in American speech may well descend from the local dialects of southwestern England, where a lot of the early American settlers came from.
