When Karl was in the institution in the beginning and the end, what was J.T. Walsh's character talking about to him?

I'm not sure that JT Walsh's character was anything more than a lot of talk. If you didn't know the movie or anything about the plot, when the girl comes to interview the patient who is a killer, it's easy to assume it's JT Walsh's character, but the real killer wasn't the guy blabbing about it.
