Something I don't understand (spoilers)

When Pinhead decapitates John Merchant with the hook and chain, why does Pinhead have to look behind himself to make sure the hook was coming in the first place?

Well I know it's added for emphasis as it pertains to Pinhead's telekinetic powers, but I don't see how it was necessary. As soon as you see him turn his head, you know what's coming and it eliminates the schock value. Oh well, no big deal.

Pinhead in the begining of the series, haven't seen them all yet, always had some kind of motion when he summoned his chains, a hand movement or a turn of the head.

except in the first...he was just staring at Frank (in larry's skin) like a child that was cut when he fell...the chains have their own mind.
