Did they all die??

It's a dream sequence dude - and they are having boat drinks!

Yep they all died but hooked up in a better place. There own heaven where no one is sick and all are happy enjoying boat drinks. You noticed that Pieces did not have leprosy.

There have been a number of posts about "Boat Drinks". Doesn't Jack Warden explain early in the movie about it? I believe he says that when (and if) they get out of the rackets and "retire", they would get on a yacht and just cruise around, sipping drinks. Early in the movie, you find out that Jimmy is saving money for a boat. His partner suggests he dip into his "boat money" to help save their business since it's senseless to own a boat in Denver. In the end they all die and the last scene is the hope that they can finally enjoy their "boat drinks" in the after-life. Unless you watch the movie carefully, or many times, I can see how this can be confusing.
