In my opinion, movies like this one do totally the opposite. Ginger looks like a person who has ruined her life thanks to alcohol and drugs, is a horrible mother, a very greedy person, cannot let go that pimp. So having such a bad and ridiculous person doing drugs wouldn't make the girl think "Wow, this character is so cool because she does drugs, such a great person does drugs, I should too!" Instead, she's watching the example of what drugs can do to someone.
Same thing goes for movies like Trainspotting. Adults fear that if their sons or daughters watch those movies, they're going to want to try drugs. But it's totally the opposite, you see people ruining their lifes and harming themselves, thaat doesn't make you curious about drugs.
Finally, I can't imagine Scorsesse bringing a double or doing some cgi work just to avoid setting a bad example to the girl.
