I also think Americans, or rather the commitee that represents them in the form of ratings are a bit uptight about pot smoking. Any drug usage is an immediate R rating I believe.
So I looked it up on the MPAA website. As it turns out they have a search engine and you can bring up the rating information and reasoning for any movie they have rated. Here are the results:
S.F.W. (1994)
Gramercy Pictures
R Rated R for pervasive strong language, scenes of brutal violence, constant drug and alcohol use and some sexuality.
So I was mostly right. Another interesting factoid I learned on the MPAA website is that all their ratings are completely voluntary on the behalf of the film publisher. Interesting.
Probably the sex scene with Joey Lauren Adams. It show nipple.
definitely because of both the sex scene and the amount of times they use the word *beep* I'm pretty sure you can't say it more than once or twice in a PG-13

Americans will give an R-rating to any movie that shows a boob.
As long as the gory disembowelments are kept to under 6, it can get a PG13.
