Movie Help

I was wondering if someone could help,
i'm stuck with these images from a movie, i cant remember the cast,
i only remember plot, somewhat anyway.

The other main character has some kinda of a cancer & only has about a few months to live & she meets this guy with whom she falls in love with & vice verse, but i think she doesnt tell him the whole truth about her situation & in the end of the film she runs away cus she feels like her life is a bout end soon & only leaves a message to her fella who's able to catch her at by big fountain by the end of the steep road where she lives in & they have an emotion conversation & in the end she askes him to turn around & when he looks back, she's gone.

Ring a bell, anyone, i _really_ need to know what movie this!
