Questions about Mark Sway

I have a few doubts.

1) Did Mark Sway really pretended that heart attack? The doctor said «Jesus Christ, this kid's heart is about to explode!». However, suddenly he is okay? Is it possible to pretend such thing as a heart attack? And HOW do you do that?

2) After pretending that heart attack he gets chased by one of the bad guys. When he locked the bad guy why didn't he just call the police to arrest him, taking this perfect chance to do so? Sooner or later, someone would come there and the bad guy was gonna be released. Either that or the bad guy would find a way to get out of there and would continue to chase Mark.

«I was on the fire escape! I saw ya!»

1. It wasn't a heart attack. It was an attack of post-traumatic stress disorder (like Ricky). He tells Reggie on the phone that he ran in place for an hour in his cell before faking the attack. That's why his heart was racing and he was burning up when they examined him at the jail. I admit that the book does a better job of building up to the attack, but I don't think that would have worked at this point in the movie. The movie was moving too fast for a slow build-up to the attack, and I think inserting such a slow build-up would have detracted from the movie.

2. My guess would be that he couldn't call the police because he was on the run. Although one of the mafia guys was locked in the freezer, he knew others might still be after him. If he called the police, Roy and the FBI would find out where he was, track him down, send him back to jail, and eventually keep trying to make him reveal the location of the body (which he figured would pretty much guarantee his death). Remember at that point he didn't believe he could trust anyone (except Reggie), particularly the police.
