At the end, when he's in the car about to get out, he lights his cigarette. Where did he get that lighter from? lol Remember when he first started driving, he said he didn't have a lighter and had to use the one in the car. She threw it out the window.

Yeah, it was all his fantasy. Notice how after he is shot it cuts back to him in the car with his cigarette still unlit. He pulls it out of his mouth just before deciding to get out of the car for real.
