The mini-series does not touch it at all, and the book mentions it only in passing, but I gather that the rest of the world was wiped out as well.

Flagg is aware of others like him in other places, Iran is mentioned, and I believe that Russia and China were as well.

Then you had General Starkey telling his subordinate Len Creighton (or Carsleigh) to release the vials of the virus behind the Iron and Bamboo curtains. His attitude, as quoted in the book was 'share and share alike'.

So, yes; the world got it as well. This does not preclude the possibility that there were other communities like Boulder and Las Vegas, but they were the only ones specifically mentioned.

One of the biggest flaws is that nothing is mentioned about survivors in the neighboring countries of Canada and Mexico. Some of the characters in the book talk about fleeing to Canada or Mexico, but what about the survivors already there? No mention of Canadians or Mexicans being drawn to Boulder or Las Vegas.
