Question about the 'knocking' *spoilers*

I don't know much about ventriloquism, but can you really make it sound like the knocking (that Julia heard while STANDING right behind the door!!) is coming from the other side of the door....
...when its actually coming from a distance?? That was wild!!
And the part where he blends into the darkness of the hospital room. Creeepy!
Guess he never had to cough or sneeze either, lol.
