I didnt understand the joke about the woman who coughs. can someone explain?

One of the most beneficial things I’ve ever learned is how to keep my mouth shut.

AFAIR
- a woman goes to see a doctor because she can't stop coughing
- the doctor gives her a pill that she swallows right away
- then she asks "What is this pill?"
- the doctor says "That's the most powerful laxative..."
- she is surprised, "What? A laxative for a cough??"
- and the doctor says "Yes. Just try to cough now!"

If she coughs she'll defecate, she therefore has extra incentive not to cough.

Or maybe she will be too busy defecating that she'll forget about the coughing.

You need to say the punchline twice to understand its meaning in the movie.
