Why did Nadine leave the show?

If I remember correctly, BB was back using pain killers etc. Her substance abuse caused problems on the set. The actor who played Quentin left the show, if I remember right, because BB flashed him. (lifted her shirt) His mother pulled him, which prompted the sudden change to the older taller Quentin. I believe that Julie White left the show for similar reasons.
