First of all, he should have killed her to eliminate her as a threat. He left Vincent alive and tied up and look what happened... a clown died. If John had been killed, then Sabrina might have been freed and once again become a threat.
Backseat? Only Sabrina was in the backseat at the end. John killed Mr. Forget by slamming his head with a steel door. I think the sound effects were supposed to indicate his death. The last we see of him is when the flight attendant opened the door to the trunk and his body fell out.

When Cutter left the guy who played the flight steward alive instead of slicing his throat, I immediately knew this was a huge mistake. Why he would leave someone who could potentially come back as a threat alive in such a situation was absurd.
