that the British health care system didn't cover gender reassignment surgery at the time this movie was produced, otherwise it would have been a big plot hole, no pun intended. I am, of course, also assuming that the procedure is covered as of this posting. Perhaps somebody in the know could clarify the facts.

It was covered at the time also because British fashion model, and transsexual, Caroline 'Tula' Cossey, got her surgery through the National Health Care in England back in 1979 or 1980.

Maybe Dil didn't want it, though.
