Yeah but doesn't he say, when the girlfriend gets that skewered rat and subsequently the mother, that he thought they were from the brother of that man they chased down and killed at his hideout?

Given that the mom and girlfriend were mutilated in a somewhat similar manner, I assumed it was that guy.

Yeah, I just saw this last night and I think it was the brother of the guy they killed at the hideout. That guy was Italian and had mob connections. They were pretty much hunting almost everyone associated with the main killer. Like that Valerie girl gets a dead rat in the mail I think. That's usually a heads up from the mob that she's next on their hit list a la Godfather (horse head in the bed) and also I remember a charactor from Taxi Driver talking about how they send dead birds in the mail as well.

I felt like it was explained pretty clearly in the film. When Valerie receives the skewered rat in the mail Ben explains that he killed an Italian man at the hideout named Giovanni. He says it's a warning from the man's brother and even says, "I knew he'd turn up someday." Then he tells Valerie not to worry because they're "small-time".

It's implied that there's a connection to the Mob and that they're revenge killings.

They were probably shot by the italian mafia. In the middle of the movie, after the shoot-out in the warehouse, Valerie finds a dead beaver. The psychopath tells her not to worry and that he knew that they would do that.

MAJOR SPOILER:

Later in the movie Ben finds Valerie dead on her bed with a trumpet in her anus, which probably is the italian mafia again. At this point Ben wants revenge...
