Visible booms mics etc.

Quick question.

Is the amount of visible booms and mics that are visible intentional or not? It seems to happen fairly regularly.

Not really intentional, but an issue they were fine with ignoring as long as the scene was funny.

Garry Shandling has said that he went with the funniest take, technical mistakes or not.

There was one scene where the boom was so obvious for so long that it really bugged me, and then a pan backwards showed that it was a light fixture. This was a scene that took place in the general office area where all of the assistants have their desks.
