She is now holding Buddy's watch fob in her hands instead of wearing it around her neck. As she testifies she is sliding each link through her fingers as a devout Catholic might do while saying Hail Mary as they slide rosary beads through their fingers.

It says so much about Idgie and her relationship with her much beloved older brother, Buddy. Her hearts in the right place as she lies and the only person she feels she seeks to be penitent with is Buddy. After watching her I could imagine over the years that while Idgie was tramping through the woods all those years she was confiding, confessing, and sharing her life with Buddy.
