The one that forcefully bumps into her and calls her a fat bitch? That seemed completely random and unconnected to anything else in the movie.

Unless it was a setup to the scene a few minutes later with the two obnoxious hos.

I want the doctor to take your picture so I can look at you from inside as well.

I think it's a setup of how she's never lost her temper because it's bad manners and also a buildup to her crying because nothing in her life is right and Ninny reveals she's going through menopause.

That can randomly happen too. My mom was practically assaulted by a young man at a gas station. No reason for it.
