He wasn't imaginary

He was there just invisible. When someone has an imaginary friend, it's just in their imagination obviously. In DDF, what Fred does, actually happens like the dog poo on the floor and furniture and causing Elizabeth to act crazy in the restaurant and cutting her hair.

I always assumed that Drop Dead Fred was imaginary and that really Elizabeth did the dog poo, cutting her hair, etc... and just blamed it on Fred. He was a metaphor to excuse her behavior and to give her an excuse to do things to spite her Mother.

But the way she acts at the restaurant maybe Drop Dead Fred isn't imaginary after all.

This would also make sense that the new little girl is also calling him Drop Dead Fred. If he was truly imaginary, then his name would have changed based on the girls own imagination.

if he was just invisible, then why did taking memory pills hurt him?

Because those pills numb a part of your brain which makes you able to see imaginary friends ...
