This is true, but she says it when she's in the physician's office about to be examined. If she said her real age, she would have had to have her mother present. She didn't want that so she lied.
Just to buttress what I said in my last post about the age of Ryder's character, there's this from Wikipedia's description of the movie:
"Mermaids is narrated in the first person by Charlotte Flax, a 15-year-old girl living with her mother..."
I am sure Charlotte even says that she is 15 when Rachel is teaching her to drive.
