I was a little uncertain of that myself. It can be interpreted as a flashback, except for the fact that he tells his wife that he was dreaming about living with Jez and talked about being "on fire from ice." So this is in fact not a true flashback but a dream within a dream. That being said I'm sure it's meant to represent a real time in his life in which he was happy with his wife and his son was alive.

I'm sure the part where he tucks the little one in was part of a memory (at least I think I'm sure). But I don't think Sarah visiting him in the hospital happened. I think that was part of him "letting go" of her by getting some validation that she still loved him.
