What was going on in this hospital? Body parts everywhere a doctor injecting a long needle into his forehead. The doctor telling him he's dead already. What was the significance of the hospital scene as well as the chiropractor busting in and "saving him"?
The scene is significant because it's the first time in the film where we're told explicitly what is happening to Jacob, i.e. that he's already dead (or rather, in the process of dying). The doctor represents the part of his subconscious mind that has some recognition of what is really happening to him behind the false "reality" of his dying dreams and hallucinations.
Louis the chiropractor represents the side of Jacob's mind that's finally willing to accept his own pending death. He gently informs Jacob of the truth by implication, rather than starkly confronting him with it like the surgeon in the "purgatory" hospital.
