The beginning of the film was originally planned to have be a continuation of that last scene. Dan injects Meg with the reagent and she comes alive with relatively little trouble. They embrace for a moment and then Meg's starts convulsing; the muscles of her entire body spasming. She dies again, a permanent death this time.
As Dan cradles her body, Herbert West walks in. Covered in blood, but otherwise in good condition, he answers Dan's unspoken question: "No, Hill didn't kill me"...(a small smiles forms on his face)..."he didn't have the guts."

Unfortunately, this scene was never fully filmed and editted. It never made the final cut of the movie. A rough cut of it is available on the DVD.
The actual movie begins a few months after the Miskatonic massacre. West and Cain have been able to become full-fledged doctors. Cain has become West's partner in his bizarre experiments.
Meg is just dead, but West has kept her heart in storage (to manipulate Dan later on).

also, another add on to that note about the deleted scene, the reason Meg dies again right after being injected (as Herbert explains) is because the serum won't work when she has a broken neck

"...the serum won't work when she has a broken neck"

I don't buy it. Rufus had a broken back and Dr. Hill was a talking head for gods sake.

And why is her heart the only thing that's left?

well I quess it's because Hill's head was re-animated as a separate part and not a whole and the broken neck causes her to die all over again.....as for the heart I'm assuming she was cut up and studied because she was a twitching corpse.....sparked some scientic curiousity...there were probably other part of her around in Dr. Graves collection but West has no interst in using them as part of creating the perfect woman.

West didn't keep the heart to manipulate Cain. West found the heart in the storage room while he was searching for good parts and was genuinely surprised it was there (so was the audience, 'cause it makes no sense). THEN he got the idea to use it for the "new" body.
