Two screenshots and questions...blooper, and a mirror on a roof?

How does a blooper like this end up in the final cut? I've never understood it. Why didn't anybody make them remove it?

Is there a mirror behind Jeff?? That does not look like a real background to me...and it's warped like a bent mirror.
