If you watch the final big fight scene, you can see where VD took out that stunt man's eye.

shareOk, Van Damme opens an attack on one of the Flesh Pirates. He does a high outside crescent kick and then slashes towards the seconds guys face with a long Bowie knife. You'll know when it happens because it's in slow motion and the guy he accidently blinded is black. It's the very first fight movement in the end of the film right after he jumps off the platform.
