Prayer by Kelly Martin in one of last episodes....

I am searching for the source of a prayer being read out of a book by Kelly Martin in one of Life Goes On's final episode. I have the prayer, but am looking for which book she was reading it from. Any help will be appreciated. The prayer reads as follows: "For the blessings You have bestowed upon us, For all the days we've had together and all the days to come, For the joys and sorrows that bind us ever closer, For the trials we have overcome....And for teaching us that we can do no great things, Only small things with great love, We thank You. Amen." I know the final sentence is from Mother Theresa. Again, what I need is the book title and/or author. Thanks
