What was with Mr.Murray? Spoilers

Anyone have a more extensive idea of what the deal was with Mr. Murray/The Butler? Was he just genuinely in on the whole scheme or just plain insane and manipulated into it? He mentioned previously that he was a patient of the doctor and seemed to generate two different personalities (more gentle, sweet one and the other a more sadistic one, particularly at the end before he's killed). Perhaps, since he was a patient, he had a split personality disorder or the like!?

Definitely a patient. a basically a psychopath. when she biffed him I said ... good girl! LOL!!!

He was just a tool in on the plot of his employer who probably would have given him a big x-mas bonus for his role (he also admitted that he was the one to strangle the sister in the car at the beginning-remember, he said he used nylon cord because there would be no blood).

It was great seeing all three evil people get their comeuppances!
