He killed Lester, so he doesnt mind killing.

Did he not want to kill Brad and Teri cause they were kids? They were not exactly "civilians".

He wasn't part of the killing of Brad jr's gang either .

I think it was because Brad Jr. was DEFINITELY family (there seemed to be a question as to whether "Tommy" was really Brad's son (with Brad Sr. even calling him a "bast*rd" at one point in the movie)(?) That's just the idea I got out of it.
It really was quite evident he had no problem killing (even lighting up a smoke, while holding Lester underwater, to drown).
