I think there's a French expression, something like "he has hands everywhere", meaning he flirts a lot.

Maybe there's a similar Italian saying.

No, there's not. I'm Italian and that sentence (in Italian) puzzled me too.

I think she means that the doctor is very "grabby". But also he gets a pass because of his role there. They're not supposed to think of him as a man the same way as if he was a priest. But for someone who isn't supposed to be a man, he seems to have a lot of hands. It's a goofy joke, but one I could imagine a teenage girl saying.

It's pretty obvious that the doctor is known for engaging in inappropriate touching of the girls. He's being compared to an octopus-like creature.
