I mean, I liked the movie and all, but I feel like I missed a lot of the plot because I couldn't understand what the hell he was saying most of the time (and sadly, the DVD does not offer subtitles). Am I alone in this, or does anyone else find him difficult to understand?

How old were you the first time? Maybe you didn't have enough of an ear for accents yet.

I will agree with the OP, and I can usually understand things pretty well. I mean, I can understand everything Clarence Nash said in Donald Duck's voice for crying out loud!

I'll have to look at my copy of the movie, but I was able to find subtitles for my DVD. I also think they may have had him speak that way intentionally? Anyhow, it's a GREAT movie

No problems here... It's a heavy OTT Italian accent, but I understand Lithgow's 'dialogue coach' was actually his on-set hairdresser/make-up artist or something, whose actual accent is faithfully reproduced here...

Interesting! I had few problems w/the accent, but I usually don't (studied foreign language in college). Still, I assumed that Lizardo's babbling and angry spluttering were supposed to show how nuts he was and frantic to obtain Buckaroo's overthruster. He was meant to sound like a whacko, right?
