Near the first two deaths, there is this creepy as hell doll.
Now, but what I put together it was something to throw us off, but it confused me.

Can anyone help?

it was just a warning from the killer that she is unhappy, and that they were going to die if u see the doll. It was also a sign, wherever the doll appeared, that person will die!

But the doll does not appear before all deaths - only the first couple of them.
This may have something to do with the delays in production.

Right. After all, the doll first appears next to Amanda's bed and then is taken by the killer.
