Well, they had both just been given the talk of staying cool by the prison officers, so that was still in their minds.

Fight it out?
Paco wanted to kill Mick, not just fight him. That was something that needed to be planned out.
Doesn't need to be planned. out. All Pace had to do is beat O'Brien unconscious and keep smashing his head on the floor or just strangle him to death.

You make it sound so easy. Problem is O'Brien is going to be fighting back and fighting back hard.
Other inmates will be going in and out of the shower. As soon as any of them see the fight is going down there's going to be lots of whooping and hollering and other inmates running in to see it.
The guards who are full aware of the bad blood between Paco and O'Brien are already going to be on alert whenever the two get near one another and probably monitor who is going in and out of the shower. The fight would most likely be broken up before it even got serious. Then both inmates would end up in the hole.
Killing Someone In Jail 101:
1. Get your hands on a shank.
2. Figure out the time and place to hit your victim.
3. Do as much damage as you can in as little time possible. (aim for arteries)
4. Get the hell out of there and dispose of any evidence linking you to the crime; weapons, blood on clothes...ect.
5. Keep your mouth shut. Resist bragging about it to anybody.

It might be easy if you were fighting a much weaker opponent, but those two were pretty evenly matched.
Paco couldn't get the job done...despite the advantage of having a shank. Do you think the outcome would’ve been any different had Paco attacked O'Brien in the shower room using only his fists?
Because the movie would've ended right then and there. Mick told Paco in the shower room that he wanted to make good time and not extend his sentence. That just gave Paco more time to ride Micks ass, and make the other inmates choose sides. So if/when a fight broke out, the inmates most likely would have stopped the guards from interfering, just like they did at the end by blocking the entrance.
