The whole movie is contrasting different levels of crazy - it's socially acceptable for there to be crazy elements in our society in certain venues. The punk club - the Sic *beep* are singing "Chop up your mother" and, in the context of a punk rock song, it's just (tasteless) entertainment. But on a flyer through your door or coming out of a politician's mouth, it becomes worrying.
"Normal", "sane", "straight" people are fearful of the mentally ill, yet are also constantly reaching for that release from the tether to sanity. They go to punk rock shows, or they take drugs, or they otherwise seek an escape (the girl at the end is clearly on drugs and at the show).
The end is unclear because we have no idea if Jack Palance has found a home and can be himself, or if he still presents a danger. Is he going to shoot the girl? Does she have ANY idea what's about to happen?
It just leaves you wondering about the concequences of taking a leave of your senses, and weighing up the pro's and cons of being "sane" or "insane".
This is such an insightful post. It makes me appreciate the flick more.
