Hi all-
Ok, so I know that Fell was actually Kane's brother and the commanding officer, but who were we supposed to think he was before the big reveal? I was a little distracted at the beginning when Kane first arrives and meets everyone, and I couldn't really keep track of all the men throughout the movie (they all kind of looked and acted the same to me). I remember him meeting someone who says he's a doctor, but turns out to be a patient. Then he meets someone who is not wearing pants but who appears to be an actual doctor. Was Fell one of those guys? Did he arrive with Kane? Or had he been at the castle for a while? Was he acting as a guard there?

Fell was the chap without pants on at the start (he refers to his "pants" several times in the film)

Fell was simply "acting" as the Doctor - whether he was an actual Doctor or not is unclear - as part of the whole "act" for Kane.

It aint like it used to be, but it'll do

Fell later states that he actually is Col. Kane's psychiatrist. He was posing as the facility's medical doctor while following up on his brother.

To kind of add to your point, Fell was in command of the institution and the head psychiatrist. He convinced the military command to send his brother there so he could personally see if he could fix his brother.
