One thing I don't get---


How come at the beginning of the movie, when Dr. Wheeler wants to see the charts and the head of Anesthesia gets all offended and says something like, Don't you think if there had been something that linked the cases together we would have seen it?
Then it's all too obvious when Dr. Wheeler finally sees the charts that all the cases occurred in O.R. 8. So how come no one saw that? Was the head of anesthesia in on the conspiracy? And if he was, wouldn't there be more people on committees and boards that have had to see these charts?
I didn't really understand that bit.

Its never exactly clear, but Im sure that Dr. Deorge (The Chief of Anesthesiology)is, and the hitman of course. The whole staff at The Jefferson Institute. You would assume maybe some maintenance people, since the tanks and wireing is all over there workspace.

It's a deus ex machina in that, yes, someone should have spotted OR8 as the commmon denomiator but only Wheeler did, after Greenleigh died and Wheeler started poking about. It's not a hole at all, it's a polt device designed to ratchet up the tension - Wheeler sees it, and thinks at first she's going mad, but then knows she's not. Good plotting.

He was blinded by his own ego. He can not accept the possibility that something nefarious may be happening in his department on his watch. 'It's just the nature of anesthesiology. Here and there patients have an adverse reaction to the drugs and are rendered comatose.' He was not actively involved in the conspiracy, but his willful blindness allowed it to go on for years (until Dr. Wheeler came along).
