How would Katherine and Robert have not noticed the 666 birthmark when they had Damien as a child? They had him as a baby, before he had hair. Surely they would have noticed the birthmark from the very beginning?

It could be as simple as they weren't looking for it. There is also the chance that it may have been very faint at first, then got darker as he and his power grew.

They thought that it was 999 so they weren't worried about it.

When they 'adopted' Damien from the Priest and he was handed over to Robert, you'll notice that the baby had a full head of dark hair, so the 666 birthmark would not have been visable to Katherine and Robert.
