Only 1 fat person ?

Sure. Not too many ugly ones either.

That society not only prized shallow values but had the technology to alter appearances through technology.

Youth was the altar upon which they worshiped in a cultural sense. Indeed, the book had life ending at 21, not 30. What would our world look like today if we had cheap plastic surgery and drugs that could keep you slim and ageless? In fact, look at all those infomercials that try to sell you promises of exactly that. What if it could all be real? No fatties or uggos for sure.

1. Everyone in the city is under 30 - younger people tend to be more active and they usually burn more calories. I could eat whatever I wanted without putting on weight when I was younger - today... not so much.

2. Drugs seem to be a common thing in this world. Maybe they've invented some sort of "stay-slim"-pill?

3. Plastic surgery seems to be pretty much perfected and far from the unpleasant and risky procedure it can still be today. No reason to doubt that they've also perfected and simplified liposuction.

4. With everyone being this young and attractive and with things like the "loveshop" and that cross between Tinder and a ST-transporter around, it's pretty easy to assume that everyone spends most of their time fu*!ing. Which means even the ones not interested in sports get some sort of regular exercise.

Food is probably synthetic or refined. Lots of sex and emphasis on looks.
