Missing footage from German DVD

I've read that the German DVD is uncut except for about a second from the murder of the asylum director (I think I read that 40 frames are missing, not cut for this release, but previously lost, presumably forever). Well I've just watched it, and I have to say that the film is pretty tame. The brain surgery part is great, although I don't think this was ever cut anyway. All the previous "cuts" like the artery clamping and eyeball popping are very mild when finally seen.

But the murder of the director is the most disappointing. There's a famous still from this film of the monster actually shoving the broken bottle into the guy's neck, but apparently the only cut was the aftermath of the attack, with the sight of the director's bleeding throat being shortened. So what happened to this picture? Was that a publicity shot never put on film?

I guess it must have been a publicity still for the movie. Kind of like the publicity still used for the British horror film, The Vault of Horror (1973), where the actors from the film are seen standing in a graveyard with skull-like faces. This scene has never been seen in any version of the film.
