I swear I've seen Shelley Duvall, she is coughing in one of the escenes where the killer tells Scott how he killed his second victim.

I had the same reaction. She isn't credited for The Hospital and I think it's just an actress that looks like her in that brief moment.

We're not alone: There's a similar post about The Hospital on Shelley's IMDB board too.

I'm pretty sure it's her. Two people with that unique look...nah.

But, i thought Dr. Spezio was Lee Horsely

That is absolutely not her. I know who you are talking about though, her hair looks like Duvall from The Shining, it isn't her though.
