It's been a while since I've seen TADP (and I missed the first half-hour). But was Dr. Phibes right--did someone screw up, or was he just crazed with revenge and all...?

His wife was just too badly injured to survive. Driven insane by her death (and his own accident), Phibes just wanted someone to blame.
