So I guess these is one of these things where you're simply are not supposed to know who is making McClain turn around just before the credits start to roll. But seriously who could it be? The only two female characters in the movie are dead. We never get to see Ellie's body but the police is there not long after she gets killed and they do find a body and confirm it as being her on the news. Are we to assume her death was somehow staged or that the woman that put together the heist somehow survived their quite clear on-screen murders?

No, that's just McClain's conscience-he's haunted by Ellie's memory, (and the promise he made to her.)

Was it Ellie's voice? It could have been an entirely different person that knew him before and recognized him.

No. In my opinion, it was Ellie's voice. Or put it this way: it was McClain's (Jim Brown) conscience bothering him. And like the poster stated, the two main female characters of the film were killed. So i don't believe the makers of the film would just bring a totally new character out of nowhere at the end of the film.
