According to trivia on the AMC Monsterfest version I own, this film was originally called "Dementia," but another, earlier film with a cult following already had that title. To avoid confusion, it was decided to add "13" to the name. Because this was during the time of multiple run horror flicks competing against one another by employing various gimmicks, it was assumed the new title would build in a cult following whenever the film played on the 13th of the month. Decades later, the "Friday the 13th" series would work on the same assumption.

SPOILER warning!! please don't read if you did not see the movie:
I think '13' refers to the age the killer had when he started developing insanity, due to what happened then (there are two flashbacks of this in the movie). Also, another character says clearly 'he had only 13 when it happened'.

This does'nt mean the story of the title it's not as the earlier post tells (I think I recall Coppola telling that same story in the "Inside Acto's Studio" interview or somewhere else).

That's a pretty good theory, I think it holds a lot of merit.

I think the title is pretty clever. Dementia, which one of the main characters in the movie suffers from, and adding 13 to it, which is considered by many to be superstitious and/or unlucky number. The title fits the movie well.
