The Archangel had Schizophrenia. Schizophrenics can have sporadic behavior. Sometimes they can be violent, but not all schizophrenics are violent. It depends on their history. I saw the scene as a metaphor to Gabrielle's struggle with her faith. The scene is also an allusion to when Jacob wrestles with God in the bible. Jacob had a spiritual crisis, and all he wanted was a blessing from God. Throughout the movie, Gabrielle wanted an affirmation that her religious devotion would make her happier, or a better person. Traumatic episodes, such as this fight and a few others, makes Gabrielle question if the sacrifices that a nun makes, such as helping someone who is dangerous, is really better for her life. Also the Archangel was Gabriel, and Sister Luke was Gabrielle, another point for Gabrielle's inner struggle with her identity and faith.

The Archangel was said to have schizophrenia. She would not necessarily have the same diagnosis today, as labels have varied considerably. Violence and delusions do not determine a diagnosis.

Her violence was probably the result of certain aspects of her delusions, but even today it would be a hard call. There's no brain disorder which consistently (or even frequently) results in violent behavior.

I got the impression that whatever her diagnosis was, the nun's habit would set her off (hence the nickname). The two nurses in regular nurse uniforms rushed in with no problems.
