In Jupiter Ascending

I did not care for him in this movie. He looked, talked, and moved as if he was in constant pain. I couldn't honestly tell if that's the way the character was written or just the way he played it

as i understand the story, balem was in constant pain, indeed.

he´s killed his mother (with whom he´d had an a bit special connection) - and during this murder caine (who was her bodyguard then) hurted his (balem´s) throat; wherefore balem needs that stiff neck collar and speaks only whispering. maybe there were some more traumas at his body, but surely in his soul. another result of that incident is caine´s loss of wings.

See I somehow missed that. Do they talk about it in the movie?

somewhen caine tells about the loss of his wings (without telling names, but mentioning the circumstances and the throat- trauma he caused then), later anybody tells that balem murdered his mother - and balem´s voice is damaged and he needs that collar. one must put the pieces together to get the whole picture.
