He could have gotten it from smoking but while it would have increased his chances of getting pancreatic cancer, there are other risks and causes such as pre-existing medical conditions, dietary factors or family history. Cigarettes as a cause is likely but not certain - without knowing his extensive medical history, it's impossible to say with absolute certainty.

He was a horrible nasty drunk, he used drugs, and he smoked 7 packs of cigarettes a day. Cocaine use, alcohol and all those cigarettes...come on now, that is not a healthy lifestyle. Just because you saw him do yoga in Road House, doesn't mean he was all about clean living. He wasn't. He just tried to make the world believe that he was all about that, but instead, he abused his body with drink drugs and cigarettes, and crashed a plane all drunk and got away with it.
